I used the fab work of Jamie Kidd, which you can purchase from CILM.
(Please do not use this artwork without a proper license from CILM!!)

Mask and Font of choice. I used Ephesis Font, which is a pay font and I can't
supply it.
FILTER USED:
Eye Candy 4000 Gradient Glow (OPTIONAL)
Open template, Shift D to duplicate and close original. Highlight background,
copy and paste paper of choice (I used paper 8 ), apply mask, merge group.
Duplicate mask layer, merge down, resize by 105%.
On Raster layer 1, Selections-Select All-Float-Defloat, copy and paste paper
of choice (I used paper 7), Selections-Invert-hit delete on keybard-Selections-
None.
Finish layers out, using papers of choice. On my layers, I also added a
gradient glow - 3.00 fat.
On Frame layers, I added an Inner bevel, screenshot below.
Click inside frame, using magic wand. Selections-Modify-Expand by 2. Copy
and paste tube, resize if needed, drop shadow. Hit delete on keyboard-
Selections-None. Do the other frame the same way.
Copy and paste wrapribbon3, resize to liking and drop shadow.
Copy and paste flowers3 resize to liking, drop shadow. Duplicate layer,
Mirror and merge down.
Copy and paste pearls1, resize to liking, drop shadow. Duplicate layer,
Mirror and merge down.
Copy and paste embroideredbutterfly, resize to liking and drop shadow.
Add copyright info, watermark and name. Save as png.
